Posting Trips

Forms and Reports
Waiver Paper or electronic waivers are accepted by the AMC. If you choose to use paper, be sure you have the updated waiver that specifically lists COVID19 as a potential hazard.
Trip feedback Please send this form to participants after completing a trip to get feedback.
Trip Report Please fill out this report after completing a trip.
Incident Report If a participant is injured or a near miss occurs, this is the form you need. Please remember that an Incident Report should be completed whenever an injury or incident significantly affects a participant’s or guest’s experience.
Examples include:
- Medical care was sought or recommended.
- The participant left the program or facility earlier than expected.
- Non-AMC assistance (e.g., search and rescue, law enforcement) was involved.
- A near-miss occurred.
- Special precautions were taken due to a minor’s involvement or potential future inquiry.
- A significant interpersonal, behavioral, or psychological event occurred.

Leadership Status
Wilderness First Aid (WFA) Policy, including information on reimbursement.
Co-Leader Self Evaluation Co-leaders should fill out a self-evaluation shortly after the trip is done. Self-evaluations are included with a leadership application.
Leader Evaluation Leaders should fill out an evaluation of co-leaders shortly after a local, three, or four season trip. These evaluations will be included in a leadership application when the co-leader applies for leadership.
Leader Application Fill this out when you think you are ready to move from co-leader to leader status.
Finance
Pre-Trip Budget Calculator Use this tool to calculate the participant charge for a trip. Once you’ve filled out the fields and clicked submit, you should get an email with the participant cost.
Post-Trip Budget Calculator Use this tool to calculate what you should give back to Young Members after a trip, or how much each leader should pay. Once you’ve filled out the fields and clicked submit, you should get an email outlining costs.
